About the Report

Indonesia Health Insights is a quarterly report prepared by Halodoc to present an in-depth analysis of health trends in Indonesian society. This report is designed not only to highlight clinical data or disease prevalence but also to explore the social, economic, and health behavior dynamics that affect daily life.

The 2025 edition, themed “Wellness Warriors in Silence”, highlights the vital role of mothers as guardians of family health who often have to sacrifice their own well-being, conducted by Halodoc with the support from YouGov as research provider.

01. SOCIO-ECONOMIC PERCEPTIONS

Imbalance Between the Actual Role and Recognition of Mothers

Although holding control in many important decisions related to family health, the role of mothers is rarely considered as a strategic role. She is seen as executor, not director. Her decisions are often considered instinctive, not the result of thinking processes, knowledge, and careful risk evaluation.

The old paradigm in socio-economic perceptions still places the caregiving role entirely on the shoulders of mothers, inheriting a burden that continues until today.

02. FAMILY HEALTH CHALLENGES

Mothers =Wellness Warrior of the Family

A mother does not merely rely on instinct. The decisions she takes are based on experience, trusted information, as well as skills in managing limited resources.

Not only as caregivers of children, but also as supporters of the husband’s health and the main companions of aging parents. They not only have to manage the budget, but also divide time and energy to ensure everyone is still served, while also understanding the different health needs of each member. With this layered role, mothers are essentially Wellness Warriors, like the CEO of Family Health who integrates preventive, curative, financial, and emotional decisions at one time.

03. PERSONAL HEALTH & EMOTIONAL BURDENS

Mothers’ Health that is Neglected 

Our research confirms that the postponement of personal health care is a common occurrence. As many as 78% of respondents admitted to postponing medical appointments or self-care in the past 12 months. The biggest reason comes from financial limitations (41%), followed by the tendency to forget to prioritize personal health (38%), and the feeling that personal health needs are less important than the needs of other family members.

When asked to assess their health condition, level of importance, and frequency of postponing medical check-ups, mothers placed non-acne skin health, dental and oral health, sexual/reproductive health, and mental health as important areas to be addressed. However, not a few of them still postponed examinations or treatments in these four areas. This confirms a gap between awareness of needs and actual practices.

04. HOPES THAT REMAIN UNSPOKEN

Hopes for Support in Easier and More Affordable Access to Health Services

The greatest need voiced by mothers is access to affordable and easily accessible health services (39%). For those who have to manage family routines while working, access here does not only mean cost, but also the ease of obtaining care.

Hopes for Emotional or Psychological Support 

Beyond the medical aspect, another strong need is emotional or psychological support (mental health support) (36%). A quarter of respondents also emphasized the importance of safe spaces to share without stigma (25%). When respondents were asked further about the pressures that added to their emotional burdens and eventually affected their mental health, various challenges faced by mothers emerged.

Altogether, these factors form a layered circle of pressure, making many mothers feel overwhelmed and never good enough in meeting all expectations. In the end, they are often overshadowed by guilt, seeing themselves as imperfect, and blaming themselves for their limitations; whereas behind it all, they have already made great efforts and hold a very central role in maintaining family health.

Hopes for Practical Support
in Chores & Caregiving

Data findings show that behind mothers’ central role in caregiving for all family members, lies a large need for practical support in daily life. As many as 32% of mothers hope that family members, whether partners or parents, or the presence of assistants, can take part in chores. Furthermore, 26% of mothers highlighted their hope for practical assistance in caregiving itself.

Equally important, when asked about the most expected form of appreciation for their caregiving role, many mothers mentioned the opportunity to take a personal break. As many as 37% of respondents considered it the most meaningful form of recognition. This shows that there is a real need for family members to share household or caregiving tasks, so that mothers have space to recover energy and maintain emotional balance amid intense routines.
